The position will require independent management of your schedule and appointments as well as completing documentation. Visits take place in the patient’s residence which can include homes, apartments and community living facilities.
Key Responsibilities:
-Conduct comprehensive assessments of patients’ physical, emotional and
spiritual needs.
-Develop and implement individualized care plans in collaboration with the IDG.
-Manage medications and treatments as prescribed by the hospice physician.
-Provide education and support to patients and families regarding the hospice process, symptom management and end-of-life care.
-Monitor patients’ conditions and update care plans as needed.
-Coordinate with other healthcare professionals to ensure seamless care delivery.
-Maintain accurate and timely documentation of patient care and progress.
-Uphold the principles of dignity, respect, and compassion in all patient interactions.
Qualifications:
-Current and valid RN license in Texas.
-Hospice experience required.
-Minimum of 1 year of hospice nursing experience preferred
-EMR Kinnser Wellsky experience preferred
-Valid driver’s license and reliable transportation
